Billing/Insurance Explanation of Benefits (EOB's)

An explanation of benefits (commonly referred to as an EOB form) is a
statement sent by a health insurance company to covered individuals
explaining what medical treatments and/or services were paid for on their
behalf. EOB forms you receive for medical care at Health Service in Morris
will state the services are provided by Boynton Health Service.

The patient responsibility for the services not paid for by the insurance
company are billed to the patient by the clinic or hospital where the bill
was incurred.

In the case of students who utilize the services of the Health Service at
the University of MN, Morris, the patient's responsibility is covered by
student health fees. Students will not receive a bill from Health Services.

Should a student be sent to Stevens Community Medical Center for lab,
x-rays, procedures or an appointment for a specialty doctor, the student
will receive a bill from Stevens Community Medical Center for the patient
responsibility portion of charges incurred.
